Head and Neck Imaging for Oncology

Introduction

Imaging in head and neck oncology assesses tumor extent nodal disease and perineural or vascular invasion and guides surgical and radiation planning. CT provides bony detail and MRI offers superior soft tissue contrast while PET CT evaluates metabolic activity and distant metastasis. Multimodality imaging supports comprehensive staging and treatment selection.

Staging and Local Extent

MRI is preferred for assessing soft tissue tumor extent skull base and perineural spread while CT evaluates bone erosion and cortical involvement. Contrast enhanced sequences delineate tumor from inflammation and post treatment changes. Accurate staging influences surgical margins and radiation target volumes.

Nodal and Distant Disease

CT and MRI assess regional nodal involvement and PET CT detects occult nodal disease and distant metastases. Ultrasound guided fine needle aspiration provides cytologic confirmation for suspicious nodes. Imaging guides neck dissection planning and radiation field design.

Post Treatment Surveillance

Imaging surveillance protocols vary by tumor type and risk and often include PET CT for early detection of recurrence. Differentiating post treatment fibrosis from recurrence requires correlation with clinical findings and sometimes biopsy. Multidisciplinary tumor boards integrate imaging into follow up strategies.
